Pull btrfs fixes from David Sterba:

 - several fixes for handling directory item (inserting, removing,
   iteration, error handling)

 - fix lockdep warning in inode number lookup ioctl

 - fix race when finishing block group creation

 - remove link to obsolete wiki in several files
