[PATCH] hd.c

	* switched to private queues
	* set ->queue and ->private_data
	* switched to use of ->bd_disk and ->rq_disk
	* folded recalibrate[] and special_op[] into hd_info[]
	* switched to passing pointers instead of indices
	* cleaned up
