Options

Made the manual pretty in HTMLHELP format. 

Reorganized and renamed the API chapter.


Docs/manual.texi:
  - Lots of maintenance.
  - Moved the C API info to subsections and subsubsections of the main
    MySQL C API section.
  - Added chapter, section, appendix intros where they were missing.
