  Remove duplicated append_escaped()
sql/sql_analyse.cc:
  Remove duplicated append_escaped()
sql/sql_insert.cc:
  Move extra(HA_EXTRA_IGNORE_DUP_KEY) to a more logical places
  Add missing DBUG_RETURN()
