Added back old LARGEFILE handling

Fixed reference to freed memory in acl_init()/grant_init()
Fixed possible memory leak. (Could only happen in very strange circumstances)
Fixed bug in ALTER TABLE with BDB tables
Updated mysql-test for valgrind
