MWL#17: Table elimination

- First code. Elimination works for simple cases, passes the testsuite.
- Known issues:
  = No elimination is done for aggregate functions.
  = EXPLAIN EXTENDED shows eliminated tables (I think it better not)
  = No benchmark yet
  = The code needs some polishing.
