database/sql/driver: API cleanups

-- add driver.Value type and documentation,
   convert from interface{} to Value where
   appropriate.
-- don't say "subset" anywhere,
-- SubsetValuer -> Valuer
-- SubsetValue -> Value
-- IsParameterSubsetType -> IsValue
