Add {{{fullhash}}} to {{{toku_cachetable_remove}}}, and rename it to...

Add {{{fullhash}}} to {{{toku_cachetable_remove}}}, and rename it to {{{toku_cachetable_unpin_and_remove}}}.  Addresses #1195.
