Remove unused code.

univ.i: Do not #define YYDEBUG, because it is only useful for debugging
the grammar of the Bison-generated InnoDB SQL parser.

row_build(): Remove type==ROW_COPY_ALSO_EXTERNALS, because it is never
passed.
